The reinstatement form will ask you a lot of health questions, because being eligible for reinstatement of your life insurance policy requires that your health has not deteriorated since you took out the original life insurance policy.
After you send that completed reinstatement form back to your insurance company, it may take several weeks for them to review it, and decide whether or not you are allowed reinstatement of your life insurance policy.
If the life insurance company agrees to reinstate your policy you must pay all the premiums back to when the life insurance coverage lapsed, to catch it up to the present date.
Your life insurance policy will be reinstated to coverage you had when you first bought your plan.
