After receiving background information on an individual, the life insurance company will ask about your personal medical history including your weight, height, date of birth, primary physician, when your last doctor's appointment was, and so forth.
A background of your family's medical history will also need to be completed.
After completing the medical history check, there will be a section labeled temporary insurance application and agreement. Upon completing all three sections, a life insurance agent must then review the application to ensure it is completed and signed wherever necessary.
